Emergency contact

Coaches carry this on the bench at every practice and game.

This must NOT be a parent or guardian listed above. The whole point of this contact is someone we can reach when the parents can't be reached — at work, out of signal, or already on the way to the hospital. A grandparent, aunt, uncle, neighbor or close family friend who could pick your child up.

Birth certificate

The league verifies age and grade before rosters go to coaches.

Nothing is uploaded here, and nothing is stored on this page. Your child's birth certificate goes straight to the league inbox as an email attachment and lives only in that inbox, where the board checks the age and grade and nobody else sees it. This form never holds a copy.

Coaching

Every team is led by a volunteer coach.

Either one gets a SportsEngine background check link right after you submit. It has to clear before a roster is released — head coach or assistant, no exceptions.
